Unexplained Infertility

Unexplained infertility is diagnosed when all standard tests are normal yet pregnancy has not occurred — advanced ART often reveals the cause.

Overview

What is Unexplained Infertility?

Unexplained infertility (idiopathic infertility) is diagnosed when standard fertility investigations — semen analysis, ovulation assessment, fallopian tube patency, and uterine evaluation — all return normal results in a couple who has been unable to conceive. It accounts for 10–15% of all infertility cases.

Moving Beyond "Normal" Tests

At Iswarya Fertility, we go beyond routine testing to investigate sperm DNA fragmentation, egg quality markers, subtle endometrial defects, and immunological factors. Empirical treatment with IUI followed by IVF with PGT-A often resolves pregnancies in couples previously labelled unexplained.

Symptoms

Inability to conceive after 12 months despite all normal tests
Normal semen analysis, normal ovulation, open tubes
Regular menstrual cycles
No identified uterine or hormonal abnormality

Causes

1
Subclinical egg quality defects not detected by standard tests
2
Sperm DNA fragmentation causing fertilisation failure
3
Mild endometrial receptivity issues
4
Subtle immunological incompatibility
5
Undetected subtle tubal dysfunction

How It's Diagnosed

Extended panel including sperm DNA fragmentation and anti-sperm antibodies
ERA (Endometrial Receptivity Array) biopsy
Detailed stimulated ovarian reserve testing
