Varicocele

Varicocele is an enlargement of veins in the scrotum that raises testicular temperature and impairs sperm production and quality.

🩺

Overview

What is Varicocele?

A varicocele is an enlargement of the pampiniform venous plexus within the scrotum, similar to varicose veins in the leg. It is the most common reversible cause of male infertility, found in 15% of all men and 35% of men with primary infertility. The pooling of warm blood raises testicular temperature, impairing sperm production and DNA integrity.

Treatment Options

Varicocele repair (varicocelectomy) or embolisation can improve sperm parameters in 60–70% of men. Iswarya Fertility collaborates with urologists for microsurgical varicocelectomy, after which natural conception or IUI may succeed for couples previously requiring IVF.

Symptoms

Dull aching pain or heaviness in the scrotum
Visible or palpable bag-of-worms sensation in the scrotum
Testicular atrophy (shrinkage)
Abnormal semen analysis (low count, motility, or morphology)
Worsening of infertility over time

Causes

1
Faulty valves in testicular veins allowing blood to pool
2
Anatomical differences more common on the left side
3
Increased venous pressure from the left renal vein angle

How It's Diagnosed

Physical examination — clinical grade 1/2/3
Scrotal colour Doppler ultrasound (definitive diagnosis)
Semen analysis before and after treatment
