Post-IVF Pregnancy Care — First Trimester
Special considerations for monitoring an IVF pregnancy in the critical first 12 weeks — scans, medications, and when to transition to obstetric care.
Your hCG is Positive — What Next?
IVF pregnancies require additional monitoring in the first trimester.
hCG Monitoring
- Day 14 (day of test): Initial hCG level
- Day 16–17: Repeat hCG — should ideally double every 48 hours
First Scan — Viability Scan (5–6 weeks)
We expect to see: gestational sac, yolk sac, foetal pole, and heartbeat (visible from 6 weeks, 100–160 bpm).
Medications to Continue After Positive Test
- Progesterone: Continue until 10–12 weeks as directed
- Oestradiol (if FET cycle): Until 10–12 weeks
- Folic acid: Continue at least until 12 weeks
- Thyroid medication: Maintain TSH <2.5 throughout pregnancy
Do NOT stop medications without consulting us.
Warning Signs — Seek Immediate Care
- Heavy vaginal bleeding
- Severe one-sided pelvic pain (possible ectopic — even with confirmed IUP)
- Signs of OHSS: severe bloating, difficulty breathing, decreased urine output
