Not Getting Pregnant Even After Trying for a Long Time?

22 Dec 2025

Why Pregnancy Doesn’t Happen Even After Months of Trying

Many couples assume pregnancy will happen naturally within a few months. When it doesn’t even after regular, unprotected intercourse it can lead to anxiety, confusion, and emotional stress.

Medically, if pregnancy hasn’t occurred after 12 months of trying (or 6 months if the woman is above 35), it’s time to evaluate fertility. The encouraging news is that most causes are identifiable and treatable with timely care.

Common Causes in Women

1. Irregular or Absent Ovulation

Pregnancy can occur only if ovulation happens regularly. Conditions that disrupt ovulation include:

PCOS (Polycystic Ovary Syndrome)

Thyroid disorders

Hormonal imbalance

Stress or sudden weight changes

2. Low Egg Reserve or Poor Egg Quality

Egg quantity and quality decline with age. Low AMH levels indicate reduced ovarian reserve, which may affect fertilisation and embryo development—often without obvious symptoms.

3. Blocked Fallopian Tubes

If tubes are blocked or damaged, the egg and sperm cannot meet naturally. Common causes:

Pelvic infections

Tuberculosis

Previous pelvic or abdominal surgeries

4. Endometriosis

Endometriosis can affect ovulation, egg quality, implantation, and may cause painful or heavy periods.

5. Uterine or Implantation Problems

Fibroids, polyps, thin endometrium, or uterine shape abnormalities can interfere with implantation or cause repeated miscarriages.

Common Causes in Men

6. Low Sperm Count

Low sperm concentration reduces fertilisation chances and may be linked to hormonal issues, heat exposure, or lifestyle factors.

7. Poor Sperm Motility or Morphology

Even normal counts may not lead to pregnancy if sperm movement or shape is abnormal.

8. Azoospermia

A condition where no sperm is present in semen, caused by blockages, hormonal disorders, or genetic factors.

9. High Sperm DNA Fragmentation

DNA damage in sperm can lead to fertilisation failure, poor embryo quality, or repeated IVF failure.

Shared & Lifestyle Factors Affecting Both Partners

Increasing age (especially after 35 in women, 40 in men)

Smoking and alcohol use

Obesity or being underweight

Chronic stress and poor sleep

Exposure to heat, toxins, or pollution

Lifestyle optimization is often a key part of treatment.

Unexplained Infertility

In some couples, all tests appear normal, yet pregnancy doesn’t occur. This is called unexplained infertility. With guided treatment and monitoring, many couples still achieve successful pregnancies.

When Should You Seek Fertility Help?

Consider consulting a fertility specialist if:

You’ve tried for 12 months without success

The woman is above 35 and has tried for 6 months

Periods are irregular or very painful

There’s a history of surgery, infection, or miscarriage

Early evaluation improves options and outcomes.

Modern Treatment Options

1. Fertility Evaluation

Includes hormone tests, ultrasound scans, semen analysis, and specialised investigations.

2. Ovulation Induction

Medications help stimulate regular ovulation.

3. Intrauterine Insemination (IUI)

Prepared sperm is placed directly into the uterus to improve fertilisation chances.

4. In Vitro Fertilisation (IVF)

Eggs and sperm are fertilised in the lab; embryos are transferred to the uterus.

5. ICSI (Intracytoplasmic Sperm Injection)

A single sperm is injected into an egg—useful for severe male infertility.

6. Frozen Embryo Transfer (FET)

Embryos are transferred in a later cycle under optimal uterine conditions.

7. Surgical Treatments

Hysteroscopy or laparoscopy can correct uterine or pelvic problems.

8. Fertility Preservation

Egg and sperm freezing help plan for the future.
