What Happens During Embryo Transfer in IVF?

5 Sept 2025

Introduction

Embryo transfer is one of the most crucial steps in the In Vitro Fertilization (IVF) journey. It is the moment when hope meets science the fertilized embryo is carefully placed into the uterus in the expectation of achieving a successful pregnancy. But what exactly happens during this procedure? Is bed rest really required afterward? In this guide, we walk you through the entire process, step by step.

What Is Embryo Transfer?

Embryo transfer is the final stage of the IVF cycle where a fertilized embryo is placed into the uterus of the woman. This step follows several weeks of preparation and is timed precisely to maximize the chances of implantation.

Step-by-Step Embryo Transfer Process

1. Cycle Preparation Starts on Day 2 of Menstrual Period

The process begins on the second day of the menstrual cycle. This marks the start of ovarian and endometrial assessment. A baseline ultrasound scan is performed to evaluate the ovaries and ensure there are no cysts or abnormalities.

2. Medications to Prepare the Uterus

Once the scan confirms a healthy start, medications are prescribed to prepare the endometrial lining the inner wall of the uterus where the embryo will implant. These medicines, usually in the form of oral tablets or patches, help build a thick and receptive lining.

3. Regular Monitoring via Ultrasound

Ultrasound scans are done at regular intervals — typically on day 2, 7, 10, 13, and 14 — though this may vary depending on each patient’s response. The goal is to observe:

  1. Thickness of the endometrial lining
  2. Blood flow to the uterus
  3. Overall uterine appearance

When the lining reaches a triple-layer (tri-laminar) appearance, it is considered optimal for embryo implantation. The ideal thickness is generally between 8–12 mm.

4. Starting Progesterone Support

Once the endometrium is ready, progesterone injections or medications are introduced. These hormones mimic the natural environment required for embryo implantation and signal the body that it is ready for pregnancy.

5. Scheduling the Embryo Transfer

After about 3 to 5 days of progesterone support, the embryo transfer is scheduled. This timing depends on whether a Day 3 or Day 5 (blastocyst) embryo is being used.

6. The Embryo Transfer Procedure

  1. The procedure is painless and typically does not require anesthesia.
  2. The patient remains fully conscious and can even communicate with the medical team during the procedure.
  3. Using ultrasound guidance, a thin catheter is carefully inserted into the uterus through the cervix.
  4. The selected embryo (or embryos) is gently transferred into the uterus using the catheter.

The entire procedure takes about 15 to 20 minutes, followed by 10 to 15 minutes of rest at the clinic.

Is Complete Bed Rest Required After Embryo Transfer?

This is a common question among IVF patients. The answer is no. Complete bed rest is not recommended after embryo transfer. In fact, resuming light, normal activities helps maintain good blood circulation and reduces stress.

However, just like in early pregnancy, some basic precautions should be taken:

  1. Avoid heavy lifting or high-impact exercises
  2. Reduce mental stress and maintain a calm environment
  3. Eat a balanced, nutritious diet
  4. Get adequate sleep and hydration

You can return to your routine with slight adjustments, just as you would in the first trimester of pregnancy.

What Happens After Embryo Transfer?

Post-transfer, you’ll continue progesterone and other supportive medications for several days. A pregnancy test (beta-hCG blood test) is typically done 10 to 14 days after the transfer to confirm implantation.

During this waiting period, it’s natural to feel anxious. Try to stay positive, follow your doctor’s advice, and avoid overthinking every physical symptom.

Key Takeaways

  1. Embryo transfer is a precise, painless, and crucial step in IVF.
  2. It begins with uterine lining preparation and ends with the transfer of the embryo into the uterus.
  3. No anesthesia is required, and patients can resume normal activities soon after.
  4. Complete bed rest is not necessary.
  5. A pregnancy test is scheduled after 10–14 days to confirm success.
