Unexplained infertility means your basic tests are normal but pregnancy has not happened. Here is what may be going on, and how IUI and IVF are used step by step.

What is unexplained infertility?

Unexplained infertility is diagnosed when a couple has not conceived after a year of trying, but the standard tests โ€” ovulation, fallopian tubes, uterus and semen analysis โ€” are all normal. It accounts for roughly one in four infertile couples. "Unexplained" does not mean there is no cause; it means the cause cannot be found with routine tests.

Which tests must be normal for this diagnosis?

  • Ovulation โ€” regular cycles and/or a day-21 progesterone test
  • Ovarian reserve โ€” AMH and antral follicle count
  • Fallopian tubes โ€” HSG, HyCoSy or laparoscopy
  • Uterus โ€” ultrasound, and hysteroscopy if needed
  • Semen analysis of the male partner
  • Thyroid and prolactin blood tests

What could be the hidden causes?

Routine tests cannot see every step of conception. Possible hidden reasons include:

  • Egg quality problems, especially as age rises
  • Mild endometriosis, which may only be visible on laparoscopy
  • Problems with egg pick-up by the tube even though it is open
  • Sperm function or DNA problems not shown in a normal semen count
  • Fertilisation or early embryo development issues
  • Implantation factors in the uterine lining

IVF can sometimes reveal the cause โ€” for example, poor fertilisation in the lab.

Treatment options for unexplained infertility

1. Trying naturally (expectant management)

For younger couples who have been trying for a shorter time, many still conceive naturally. Your doctor may advise continuing with regular intercourse every 2โ€“3 days for a while, alongside healthy lifestyle changes.

2. IUI with ovulation medicine

Mild stimulation (letrozole, clomiphene or injections) with intrauterine insemination is a common next step, usually for 3 cycles. It is less invasive and lower cost than IVF.

3. IVF

IVF gives the highest chance of pregnancy per cycle and also shows how eggs, sperm and embryos behave. It is generally recommended:

  • after about 2 years of trying, or after unsuccessful IUI cycles
  • sooner for women aged 35 or over, or with lower ovarian reserve

ICSI is not routinely needed for unexplained infertility but may be considered in specific cases.

Does age change the plan?

Yes โ€” age is the most important factor. Women under 35 can usually start with less invasive options. From 35 onwards, and especially after 38, moving to IVF sooner is often advised because egg numbers and quality fall over time.

Lifestyle steps that help

  • Keep a healthy weight (BMI 19โ€“25)
  • Stop smoking and limit alcohol, for both partners
  • Take folic acid daily
  • Have intercourse every 2โ€“3 days through the cycle
  • Manage stress โ€” infertility is hard, and counselling can help

Is there hope?

Yes. Couples with unexplained infertility generally have good outcomes with treatment, because no major barrier has been found. The key is a clear, time-bound plan. Talk to our specialists at Iswarya Fertility, and read about the IVF process step by step.

Frequently Asked Questions

What does unexplained infertility mean?

It means you have not conceived after a year of trying, but standard tests of ovulation, fallopian tubes, uterus and semen are all normal. The cause exists but cannot be found with routine tests.

How common is unexplained infertility?

It is diagnosed in roughly one in four couples who have difficulty conceiving.

What is the best treatment for unexplained infertility?

Treatment usually goes step by step: trying naturally for a while, then IUI with ovulation medicine, then IVF. IVF has the highest success per cycle and is advised sooner for women aged 35 or older.

Can you get pregnant naturally with unexplained infertility?

Yes. Many couples, especially younger ones who have been trying for a shorter time, still conceive naturally. Your doctor can tell you how long it is reasonable to keep trying.

When should I move to IVF for unexplained infertility?

IVF is generally recommended after about two years of trying or after unsuccessful IUI cycles, and earlier for women aged 35 or over or with low ovarian reserve.
